HEALTH AND SAFETY RULES
Health and safety is of primary importance when working in the field. LMK Pipe Renewal desires that each employee be thoroughly aware of the following preventative measures: Field workers have an exposure to polluted water and refuse by virtue of the very nature of their work during the installation of the liners. The most important factor in promoting good health is to observe the standard hygiene principles of washing properly, not eating or smoking except after washing, and showering after exposure to wastewater. Workers should keep their fingers from the nose, mouth and eyes. Field workers should follow a slogan commonly used by bacteriologists:
“A good bacteriologist never places his/her hands above the collar while at work.”
The best defense against infection and illness is the practice of good personal hygiene and prompt medical attention for any injury that breaks the skin. While observance of such precautions is an important safeguard, employees should obtain inoculations against certain disease, such as tetanus and polio, as additional precautionary measures with booster shots at scheduled intervals. This type of inoculation is the responsibility of the employees and can be obtained from a personal physician or the County Health Department.
Whenever working around sewage, hands and fingers shall be kept from the nose, mouth and eyes. Rubber gloves should be worn when cleaning pumps and equipment or at any time, a person comes in direct contact with sewage water.
LMK Pipe Renewal has developed safety rules in order to ensure that all employees might benefit from the safest working conditions that can be provided. All employees are instructed to follow these safety rules.
Each employee is responsible for his/her own personal safety. Each employee is to ensure that his/her actions do not create a hazard for his/her fellow employees or the general public.
All employees shall report personal injuries, no matter how minor, to their supervisor within twenty-four (24) hours of the accident.
Buildings and work sites must be posted with various signs as required. Employees shall obey these signs.
Fighting, wrestling, throwing objects and all forms of horseplay or practical jokes are forbidden at all times.
Running is prohibited in the LMK facility, except to address an emergency.
Office employees may eat only in the designated lunch area. Eating is never permitted in any processing areas.

Smoking is allowed only in the designated area outside of the shipping department. No smoking is allowed inside the facility.
Proper techniques and protective equipment must be utilized when handling any waste byproducts. Employees shall thoroughly wash their hands after handling any such substances.
Employees shall wear clothing of sufficient protective type for all work that is performed.
Employees shall not wear loose, flowing or ragged clothing near any moving equipment or machinery.
Employees must use extreme caution when wearing rings or other jewelry, which may become caught or smashed when working with heavy objects or near moving equipment. Post type earrings are allowed but hoop or dangling type earrings are prohibited.
Employees are required to use proper lifting techniques when ever lifting or handling materials. Try to use mechanical lifting equipment whenever possible. If objects are large or bulky, employees must seek help.
All areas shall be kept clean and in good order in accordance with activities being performed.
All aisles, stairways, passageways, exits and access areas to buildings or equipment shall be kept open and passable. Materials, equipment and refuse shall not be left remaining in Aisles or allowed to protrude into aisles or passageways.
Oil, water and other liquid spills shall be cleaned up immediately or covered with absorbent material.
Materials, tools and equipment shall be placed and/or stored in designated approved areas.
Oil rags, metal, or combustible refuse shall be placed in the proper type containers, which are identified for that purpose.
Smoking, open flames and other sources of ignition are not permitted within 25 feet of any area of operation where flammables are used or stored.
A tool, machine or equipment shall be used for the purpose for which it was intended. The unauthorized use of a tool, machine or equipment is prohibited.
Defective or unsafe tools, machines and equipment shall not be used. They shall be placed out of service for repair or replacement and shall be reported to the crew leader/manager.
Report to the manager/crew leader, without delay, any safety hazards or defects in tools, equipment or vehicles owned by LMK. Correct the hazard immediately if you are able to do so.

Only authorized personnel will perform preventative of corrective maintenance, inspection, lubrication and adjustment on all LMK equipment. No safety equipment shall be removed or altered without proper authorization.
Guards on machines shall not be removed except during repairs and adequate temporary or permanent guards shall be provided before new or modified equipment is operated.
Employees are to use extreme caution when working on or near moving machinery, equipment or conveyors. Observe signs and guards placed for the employee’s protection.
No employee shall operate a LMK owned vehicle on public thruways unless he/she possesses the appropriate valid Florida driver’s license.
Each employee is responsible to see that the vehicle he/she drives is in safe mechanical condition and all safety equipment is functional (brakes, lights, horn, steering, tires etc.). Employees shall wear the seat belts at all times when operating or riding in any LMK vehicles or any vehicle used for LMK business.
Employees who use LMK vehicles are expected to operate these vehicles in a safe and legal manner. Employees who receive a traffic violation for operation of a LMK vehicle shall be responsible for payment of any fines and/or costs that are imposed on them. Employees, who receive a moving violation, must notify their crew leader or manager. Receipt of such a violation notice shall be grounds for disciplinary action, such as for the violation of any safety rule or practice
All vehicles are equipped with the proper fire extinguisher with up-to-date certification.
Vehicles will be equipped with accident forms for the proper reporting of any accident.
Hard hats are required at all times in designated hardhat areas throughout the facility. All other areas as the grounds, maintenance shop will be considered a hardhat area when there is overhead work being performed or when a manager or the Safety Committee directs that such headgear be worn. Employees will be accountable for their failure to wear hard hats.
Approved safety glasses are required around portable of fixed machinery or when a supervisor or the Safety Committee directs that such safety equipment be worn. Darkened lens or sunglasses are for outside use only, except when using welding or cutting torch equipment.
When the employees work assignment involves any risk of particles, filings or other such hazards entering the employee’s eyes employees must also wear approved shields on the safety glasses or goggles.

Employees who perform work with chemical handling, storage etc. must wear proper goggles, gloves, face shield etc. as required by the work.
LMK Pipe Renewal has a policy of providing personal protective equipment, which is appropriate for the job. Employees are required to wear the equipment, which is provided.
Employees who perform welding, cutting, burning or other “hot work” shall wear proper welding/hot work protective equipment.
Respirators and hearing protection will be used in accordance with the appropriate sections of this policy.
No makeshift ladders or devices shall be used for any climbing at LMK Pipe Renewal, Inc facilities.
Metal ladders shall be prohibited near any electrical equipment or wiring.
Only properly designated and approved ladders shall be used. Defective ladders should be reported immediately to the manager of the tool crib and be taken out of service immediately.
Ladders shall be so placed as to prevent the possibility of slipping, or the ladder shall be lashed or held in position.
All temporary open trenches under LMK Pipe Renewal jurisdiction shall be properly cribbed shored or the trench walls properly sloped to prevent sliders or cave-ins. Trenches shall be properly barricaded and properly marked.
Designated personnel on a regular basis shall inspect all portable LMK safety equipment.
Safety equipment that is defective, missing shall be replaced, repaired or recharged as necessary.
First aid kits and other safety equipment shall be inspected on a regular basis.
All permanent and portable hoists and chains shall be checked before being used and a record made of the condition of this equipment.
